Introduction to Short Break Services - Cornerstone
Cornerstone provides high quality Ofsted registered Short Break Services to meet the assessed needs of the individual child / young person from 2 to 19 years old who may experience a range of different needs ie.
- Severe Learning Disability
- Autism ( Autistic Spectrum)
- Physical Disability
- Sensory and communication challenges
- Complex and associated behaviours that challenge
In line with the governments agenda for Aiming High for Disabled Children, "Cornerstone Short Break Service" is fully flexible, providing support 24 hours a day, 52 weeks of the year, wherever that support is required.

- Community activity support
Cornerstone staff have a wealth of experience within the field of Social Care and have a sound and reliable reputation for working in partnership with parents/carers and professionals to provide a range of support to meet the identified needs of the child/ young person.
The level of support provided to the child or young person is dependent upon the full assessment of needs and the agreed package of support. The assessment will underpin a working document which will form a Care Plan and will always encompass the 5 Outcomes of "Every Child Matters".
We feel that it is vital that the child / young person, family/carers and professionals have confidence in our quality of services and ensure through meetings, that their voices are heard.
The information gained from these meetings/visits is vital to be able to provide a fully needs led service. The Care Plan is reviewed and monitored monthly with an identified key worker liaising with all. The Care plan states the goals and objectives the child / young person is working towards and how their care and health needs are to be met.
Through Quality Assurance monitoring Cornerstone gains valuable insight into the support and operation of the Service and strives to maintain a high quality of service to meet the very specialist needs of the children and young people we support.
